Medical Management Forms
Medical Management at St Mary of the Cross Catholic Primary School
At St Mary of the Cross Catholic Primary School, the health and safety of our students is our priority. As mandated by Melbourne Archdiocese Catholic Schools (MACS), we must have in place plans and procedures to manage our students’ health care needs, including situations where we are administering medication during school hours.
If your child develops a short term illness / condition during the school year (such as tonsillitis, an ear infection, common cold, school sores) and the school is asked to administer medication, a general Medical Management Plan and Medication Authority Form must be completed and signed by the GP / AHPRA registered practitioner. Therefore, it is important to have a template copy of both forms readily available when taking your child to the doctor.
Both forms can be downloaded here.

If your child is diagnosed with an ongoing medical condition (such as asthma, anaphylaxis, allergies, hayfever, eczema etc), health care need, complex medical need or requires personalised care and support, and where medication is required to be administered at school, please attend your child’s GP to complete your child’s condition specific OR generic medical management plan. The plan must be signed by the GP / AHPRA registered practitioner; not a pharmacist.
Links to the forms can be found on the following websites or a hard copy can be provided to you from the office.
